Patient Forms

New patient documents are completed via an e-sign process before services begin; however, a few forms are provided below.

HIPAA Noticy of Privacy Practices: Provides information on the procedures used by our office to protect your privacy and to insure the confidentiality of your records. 

Release of Information: This form must be completed to allow our office to share your health information with others.

MYIO works best when accessed from Chrome, Edge, or Safari. Access your account to view your upcoming appointments and complete measures assigned by your provider.

Modes of Therapy

LBHC offers in-person and telehealth services for patients, depending on insurance and other factors.

Most appointments will be scheduled in person. Specific protocols are in place to encourage safety for our patients and staff, including screening patients prior to appointments and extra cleaning procedures. If either the patient or the provider is experiencing symptoms, masks may be required during the appointment or the appointment may be held via telehealth platform. Staff will discuss the options during the screening procedure.

Telehealth appointments are available to keep continuity of appointments during times of illness or extenuating circumstance (e.g., severe weather). In these cases, patients are able to access their appointments through a secure, HIPAA-compliant telehealth platform (Doxy.me). Telehealth appointments are currently allowed by most insurance companies. Patients must be in a private location, free of distraction during appointments. Internet connections must be secure and strong to allow uninterrupted service through this means. Patients must be in the state of Texas when they connect for the virtual appointment.
